Wellness Incentives
What are Wellness Incentives?
An annual reimbursement for covered members who complete one of the eligible screening procedures on your critical illness and cancer insurance plans.
Plan Rules:

Eligibility: You, spouse and dependents who are covered on the critical illness or cancer plans

How it works:
- If you or a covered dependent get one of the eligible screenings, you can file a wellness claim
- Once approved, you will receive a check for the wellness benefit amount
- The wellness benefit can be filed annually as long as your critical illness and cancer plans are in force

What Qualifies as Wellness?
Critical Illness - Cigna
• Mammography
• Pap Smear for women over Age 18
• Flexible Sigmoidoscopy
• Hemoccult Stool Specimen
• Colonoscopy
• Prostate Specific Antigen (for prostate cancer)
• Stress test on a bicycle or treadmill
• Fasting blood glucose test
• Blood test for triglycerides
• Serum cholesterol test to determine levels of HDL and LDL
• Bone marrow testing
• Breast ultrasound
• CA 15-3 (blood test for breast cancer)
• CA125 (blood test for ovarian cancer)
• CEA (blood test for colon cancer)
• Chest X-ray
• Serum Protein Electrophoresis (blood test for myeloma)
• Thermography
